Concurrent `eth_getBlockByHash` / `eth_getTransactionReceipt` / `eth_getBlockReceipts` requests hitting that window read inconsistent state and `mined_transactions_in_block` / `mined_receipts` returned `None` via `?`, hiding freshly loaded blocks.
